Update BERT xform to return a tensor (#cond+#proc+#drug, 768) i.e. seq

of codes. Need seq for RNN instead of summing.

Add 2 collate functions 1 for CodeEmb, 1 for DescEmb.

Will need each of them to pad/reverse seq to max seq length.
